Please complete the forms linked below. You may complete the forms in any order and return later to start additional ones, however, the forms cannot be saved and must be completed in one sitting.
For ATHLETES, please complete all forms. For NON-ATHLETES, please complete all forms EXCEPT for "Athlete Biography Form".
FISU REGISTRATION PROCESS
STEP #1 - REGISTRATION, Travel, & academic eligibility (~1 hour)
IMPORTANT NOTE: This Step 1 UNIUSA registration form can be completed in multiple sessions, and your progress can be saved. Please see the note at the top of the form (see form button below) for detailed instructions on how to save and return later.
If you need to change any part of your registration at any time, please email liam@basketballtravelers.com with the changes.
WHAT TO PREPARE FOR THIS STEP:
To complete your registration, please have the following documents ready:

(For Athletes) Academic Transcript (see button below for more details)

Headshot Photo (Format: .jpg, .jpeg, or .png):

The photo should capture your head and shoulders against a solid WHITE background. Example

Do not wear glasses, hats, or hair accessories.

File Naming: Save the file as FirstnameLastname_USA_Picture (e.g., JohnDoe_USA_Picture.jpg).
Passport Information Page (Format: .pdf, less than 500KB):

Ensure that all numbers at the bottom of the page are clearly visible.

File Naming: Save the file as FirstnameLastname_USA_Passport.

If you do not currently have a valid passport, please apply for one immediately and upload a copy of your driver’s license in the meantime. Once you receive your passport, please email a copy to liam@basketballtravelers.com.
(For All Non-Athletes) SafeSport Curriculum Certificate of Completion (~95 minutes if not completed within the last 12 months):
If you have already completed the curriculum in the last 12 months, please download your certificate to be uploaded in the registration. Rename the file to: FirstnameLastname_USA_Safe (ex: John Doe would be JohnDoe_USA_Safe)
If you have not completed the curriculum in the last 12 months, click on this link, create an account, click "Catalog", then purchase the "SafeSport Trained - Not Affiliated - CS" curriculum ($20). Complete the curriculum and download the certificate. Rename the file to: FirstnameLastname_USA_Safe (ex: John Doe would be JohnDoe_USA_Safe).
