STEP #3 - ADDITIONAL DOCUMENTS

Please download, complete and return the required documents listed below. All completed forms should be submitted via the upload button below.

 

For Athletes:

  • Academic Eligibility Form

  • Athlete Biography

  • Medical History

For Staff:

  • Medical History

  • SafeSport Certification: Submit proof of a SafeSport Certification or Recertification within the last year.

WHAT TO PREPARE FOR REGISTRATION:

To complete your registration, please have the following photos ready:

  1. Headshot (see detailed instructions below)

  2. Passport Information Page

Photo Requirements:

  • Format: .jpg, .jpeg, or .png

Headshot Photo:

  • The photo should capture your head and shoulders against a solid WHITE background. Exmaple

  • Do not wear glasses, hats, or hair accessories.

  • File Naming: Save the file as FirstnameLastname_USA_Picture (e.g., JohnDoe_USA_Picture.jpg).

Passport Information Page:

  • Ensure that all numbers at the bottom of the page are clearly visible.

  • File Naming: Save the file as FirstnameLastname_USA_Passport.
